Notes

  1. The Sample/Device Name should be written as you would like it to appear on the Results Report or Calibration Certificate.
  2. In the Description and Explanations section, descriptive information such as dimensional and mass characteristics of the Sample/Device, identifying code number, batch/serial number, manufacturer, brand, model and measurement range information, especially for EHG-5 coded services, should be given. If possible, drawings, specifications, etc. containing such information should be attached to the application form by referring to this section. If a special condition is required for the storage of the device/standard/samples, this information must be provided.
  3. In the absence of a specific request by the customer for the service, calibration/testing is performed according to the relevant standards and the conformity assessment of the results is left to the customer.
  4. In case a conformity assessment for the service is requested, the assessment criteria must be submitted to TÜBİTAK SAGE before the offer is accepted.
  5. If it is a service included in the "Industrial Services List", the EHG code in the said list should be entered. If not, "Other" should be written and the method requested to be applied in the "Other Information" section should be explained. (If possible, the relevant national/international standard number should be specified). If it is requested to apply a service included in the Industrial Services List with deviation, the EHG code should be entered and the deviation to be applied in the "Other Information" section should be clearly defined.
  6. If it is a service included in the "Industrial Services List", the EHG code in the said list should be entered. If not, Other should be written and the method requested to be applied in the "Other Information" section should be explained. (If possible, the relevant national/international standard number should be specified). If it is requested to apply a service included in the Industrial Services List with deviation, the EHG code should be entered and the deviation to be applied in the "Other Information" section should be clearly defined.
  7. TÜBİTAK SAGE has determined the conditions for industrial services with the TÜBİTAK SAGE Industrial Service Conditions document and announced it on its website. All applications are evaluated within this scope.
